Five Easy Steps
Step 1: Complete and sign the easy application.
You can get our easy application and certification in two ways. Click on your choice below:

If you like, you can complete the application and certification and return it by mail or by email: AmplifyLife@ors.sc.gov
Step 2: Have a professional certifier complete and sign the certification.
Printed on the back of the application is a brief certification form. Just take it with you when you meet with a professional certifier, who can be any one of the following:
- Audiologist
- Physician
- Physician’s Assistant
- Advanced Practice Registered Nurse
- Speech-Language Pathologist
- Hearing Aid Specialist
The certifier will sign the form to confirm that your permanent disability makes it difficult to use the phone.
Step 3: Choose the product that works best for you.
There is a wide selection of products to choose from. And we make it a breeze to find the product that fits your needs.

Step 4: Provide copies of these documents.
Please DO NOT send original documents. We only need photocopies of these very important documents to verify your identity and South Carolina residency:
- Copy of a valid SC driver’s license, SC identification card, or SC voter registration.
- Copy of a page from your current phone bill showing your phone number and address.
Step 5: Send everything in an envelope
Simply put these items into an envelope and mail it back to us:
- Application Form completed with professional certifier signature
- Copy of a valid SC driver’s license, SC identification card, or SC voter registration.
- Copy of a page from your current phone bill showing your phone number and address.
