SCEDP works with organizations and industry professionals to assist consumers with applying for equipment. Organizations and industry professionals needing more than one application should contact SCEDP to receive an application packet by mail.

Important:

To be eligible, you must complete BOTH the application and the certification. After you complete the application form, turn it over and have the certification completed by a professional. Simply take the form to a professional who can certify your disability. Then send in the application and certification form with a copy of your South Carolina ID and a copy of a page from your phone bill.
